How to track IEP and 504 accommodations by class period
Keep one master sheet: a Classes tab with each period, its gen ed teacher and co-teacher, and a Students tab where each student is entered once with their class periods and a checklist of their IEP or 504 accommodations. Then build a By Period view that uses FILTER to list every student in the chosen period with their accommodations, and print or share that view with the teacher.
The tabs and columns to set up
| Tab | Columns |
|---|---|
| Classes | Period or block, days or rotation, course, gen ed teacher, co-teacher, para, room |
| Accommodations | Accommodation name, what it looks like in class |
| Students | Student (initials or ID), IEP or 504, case manager, classes 1 to 8, a check for each accommodation, notes for teachers |
| By Period view | Pick a period: every student in it, their accommodations and notes, with the teacher and room filled in |
| Log (optional) | Date, student, class, accommodation, status (Provided / Offered, declined / Not provided), notes |
Step by step
- List your class periods. One row per section, using your school's own labels (1 to 8, 4A/4B, Block 2, A day/B day). Add the gen ed teacher, co-teacher and room.
- List the accommodations. Write each accommodation once with a plain-language note on what it looks like in class, such as extended time or preferential seating.
- Enter each student once. Add the student, pick their class periods from dropdowns, and check off their accommodations. Don't copy accommodation lists into separate teacher sheets.
- Build the By Period view. Put a dropdown for the period at the top, then use FILTER (or QUERY) to pull every student whose classes include that period, with their checked accommodations.
- Add a By Teacher view. Same idea, filtered by teacher, so a co-teacher or para sees every class they are in.
- Share only what each teacher needs. Print or save one period as a PDF for the teacher instead of sharing the whole file, and use initials or IDs.

“Enter each student once. Every view updates automatically, so you don't have to retype accommodation lists for every teacher.”— Nicolette Lesniak, Ed.S., special education coach (full review). Reviewed using a free copy.

Frequently asked questions
How do I give each general ed teacher only their students' accommodations?
Filter a view by class period or by teacher, then print or save that view as a PDF for the teacher. The master sheet stays with the case manager.
Why enter each student only once?
When accommodations change after an IEP or 504 meeting, you update one row and every period and teacher view updates with it. Separate copies per teacher drift out of date.
Does this work with block or rotating schedules?
Yes, as long as the class labels are your own. Type periods like A day/B day, Block 2 or 4A/4B in the Classes tab and pick them from dropdowns.
Can I log whether accommodations were provided?
Yes. Add an optional log tab with date, student, class, accommodation and a status such as Provided, Offered, declined or Not provided.
Does a tracker replace the IEP or 504 plan?
No. It is a communication and planning aid. The IEP and 504 plan and your district's systems remain the official records.
Is there a ready-made template?
Yes. The Fieldwork Accommodations Tracker is a Google Sheets file with Classes, Accommodations, Students, By Period, By Teacher, Student Snapshot and Log tabs already built ($7.99 on Etsy).
